Program Eligibility

Child Dental Access Program

CDAP serves children though 18 years of age who reside inside Champaign County but outside Champaign and Urbana and whose family lives at or below 200% the Federal Poverty Level. For 2008 200% of the Federal Poverty Level for a family of four is $42,400. To confirm eligibility or make an appointment, contact our office at (217) 359-7404.

Mobile Dental Clinic

Our Mobile Dental Clinics serve all patients with a completed consent form. Care will be billed to Medicaid, Insurance or Private Pay. Those with financial need but no public or private insurance may be eligible for grant supported care.

Our Mobile Dental Clinics are available for adults or children at any site with 20 or more patients with completed consent forms. The Clinic is available to set up at any site throughout greater Champaign County.

Mobile Restorative Dental Care Clinics are available to help underserved groups complete needed dental treatments. For more information contact our office at (217) 359-7404.

Head Start Dental Clinic

The Head Start Dental Clinic provides comprehensive dental care to a child enrolled in Champaign County Head Start. Care is also available to the entire family of an enrolled child.
